Blue Shield of California is offering an optional in-home care program at no extra cost through ConcertoCare. The service gives eligible members access to virtual and in-person medical support while they continue seeing their existing primary care doctors and specialists. ConcertoCare provides urgent care, mental health support, medication management, lab services, transportation coordination, nutrition guidance, and chronic illness management. The program also includes a multidisciplinary care team made up of doctors, nurses, social workers, health coaches, medical assistants, and pharmacists.
